We have read and understand the attached Wait List Agreement. We agree to the timelines, procedures, and expectations.

We understand ACA will not accept unimmunized children. A copy of my child's shot records will be provided before starting at ACA and will be kept updated.

We understand that in order to be added to the Waitlist we must make a $30 non-refundable payment by clicking the 'Pay" button below or by calling the school at (918) 248-1292. If no payment is recieved then we will not be added to the Waitlist.
